Privacy Policy

It's important for you to know and understand what and how I will manage your personal details (data). This is required by law and you can remove your consent at any time. In the UK, data protection is governed by the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. I am responsible for using your personal data and following strict rules called ‘data protection principles' unless an exemption applies. There is a guide to the data protection exemptions on the Information Commissioner's Office (ICO) website. Your personal information may be stored electronically or in paper form. Those electronically held (e.g. clinical notes) will be password protected and paper data (e.g. drawings) in a locked filing cabinet. As a therapist in private practice, and handling your personal data, it's mandatory for me to be registered with the Information Commissioner's Office.

Data Held

When you first contact Afton Music Therapy, I will hold some general pieces of information. Child's Name, Date of Birth, School and Year Group. Parents Name, Address, Phone, Alternate Contact Name & Phone, Email, Relationship with Child.

This may lead to more sensitive information being stored, such as:
  • Reason for referral (behaviour issues, exclusions, loss, neurodivergence)
  • GP details and medical conditions
  • Family circumstances
  • Information from schools or other services, such as social services
  • The aims of therapy, clinical session notes, goals and outcomes: the aims of therapy

Data Sharing

Information is not shared outside Afton Music Therapy without your permission, unless the law allows me to do so. This sharing may be to local authorities like, health care professionals (NHS), schools, social services and police.

How long will I store your data

Your data will be kept and stored securely for 7 years before being safely destroyed.

Your Rights

If you wish to access this data, you can submit a subject access request by email. This means I will provide you with a description of the information held. I will tell you how long it has been held and when it may be destroyed. I will explain where the data came from, to whom it has been shared, and provide you a copy of the information. None of your data will be passed onto any marketing organisations

Complaints
